  LIFETIME MEMBERS – SERVICE NO LONGER OFFERED FOR NEW MEMBERS

1              MedicAlert membership 

1.1          MedicAlert manufactures and supplies personalised medical identification jewellery and provides an annual membership service for Lifetime Members.

1.2          As an existing Lifetime Member of MedicAlert you agree to MedicAlert holding a confidential record containing your Personal Information which is reviewed by a nurse. MedicAlert will only disclose the Personal Information held on your record to medical or emergency professionals who are assisting you.

1.3          Your membership must be renewed annually and includes MedicAlert keeping a confidential electronic record with your Personal Information and providing details from this record if required in an emergency 24 hours a day and 7 days a week.

1.4          In order to retain your lifetime membership with MedicAlert you must contact our Customer Services team on or before your renewal date each year to confirm that the details we hold for you are correct. It is your responsibility to confirm your details are correct and MedicAlert will not issue you with a reminder

1.5          All amendments which are made to your record are chargeable with the exception of amendments to your name, address, contact telephone number and the name, address and contact telephone number of any of your emergency contacts

1.6          If you fail to confirm the details MedicAlert hold for you each year then your record will be cancelled and your details will no longer be available to the MedicAlert emergency team. If your membership with MedicAlert as a Lifetime Member is cancelled this cannot be renewed at a later date

1.7          If you later choose to re-join MedicAlert, you will be welcome to subscribe to the annual membership service and our Standard Terms and Conditions will then apply

• 1.8          You have the choice to switch your membership to our annual membership service at any time. Payment for your annual membership is required and renewable upon reminder each year
