The Coalition for Personalised Care (C4PC) is a movement of health providers, commissioners, people with lived healthcare experience and their representative bodies.
Their vision is to make personalised care an everyday reality for people who rely on healthcare services as well as organisations & people providing them.
C4PC offer trusted learning resources and share their knowledge to their community. They advocate for evidence-based approaches that prioritize individuals' needs and champion Personalised Care by empowering individuals with the skills, knowledge, and confidence to benefit from it.
C4PC offers hands-on support to encourage individuals, organizations, and communities to adopt new perspectives on Personalised Care and facilitate change. They focus on strengthening communities and aiding those who are already making positive impacts.
C4PC use their collective, independent voice to advocate for and shapechange. Their close partnership with the Personalised Care Group at NHS England is geared towards achieving the objectives outlined in Universal Personalised Care and the NHS Long Term Plan.
Studies show that when patients have the opportunity to take part in the decision-making and planning of their own health and social care there are a lot of good outcomes that can come from it.
